ASN Consent and Release by Parent or Guardian

Every driver under 18 years of age must complete and submit this form along with the membership application form.

ASN Annual Parental Consent Waiver

Every driver under 18 years of age must complete and submit this form along with the membership application form.

2010 Event Entry Form

You must complete and submit this form before each event.

Event General Waiver

This form will be available at event registration and must be signed by every entrant, or by the parent or guardian of a minor entrant.

Event Waiver - Minors

Every minor entrant must complete and submit this form before each event.

Pre-Tech Self Declaration Form

You must complete and submit this form before each event. Cadet, Novice and rookie drivers cannot self-declare for pre-tech, and must submit their karts for a pre-tech inspection.

2010 License Upgrade Request Form

Use this form to request a change to a different license class.
